Your employer should do this for you if you receive any benefits in kind from them.

If you need to complete one for other employees the PAYE scheme pack comes with all the details you need to complete the form and your boss or company account should be able to advise.

The revenue will also advise if you get really stuck
carefully - the buggers will crucify you for tax if you get it wrong. Can't your comapny accountant help?
