How do you calculate sick pay when it says a maximum payment in any 12 month period?

I am struggling to know how to work this out does anyone have a form they use

Answers:
some companies only pay Statutory sick pay now, others pay salary for a given number of months or cap the payment which yours would appear to do. So they pay sick pay until that cap is reached and that's your limit
