Urban driving simulates driving through a town or city, with a lot of stop-starts and a max speed of about 30 mph. Extra-urban driving simulates driving on the open road. Combined driving combines the two, although not in equal proportions, to give an estimated overall fuel economy figure for a normal balance of driving.

You must remember that all these figures are gathered from vehicles on rolling treadmill type test-beds, and so wind resistance is not taken into consideration. As such, you are unlikely ever to match the quoted figures for you particular car. As a means of comparing the fuel economy of one car with another, such figures do provide useful information, however.
