I would go ask at your local post office first, they give them out there so they should know something or even a number for you to contact
Bring your car registration documents to the office that issues Motor Tax Discs. This office will give you a form to fill in and foe a small fee will issue you with a duplicate disc.
Contact dvla via phone its automated answers but there is a cartax helpline where you will be guided to a customer support who you can speak to meanwhile just pop a note in your car window where the tax disk goes saying disk stolan or lost its all on computer anyway so if a t.warden or policeman can check your details and a computer will inform them that you have paid your tax

apply for a duplicate too DVLA for a nominal fee
Report it lost to police then dvla.
