Im having problems with my car juddering and jerking when I change gear.It seems to be losing power as well?

sounds like your clutch is knackered, does the engine revs rise quickly without traveling any faster?if so it has.
If your car is an automatic, it's possible the transmission lockup cellanoid is shot - same thing happened to me.

Might need a new tranny.
Sounds expensive. May be the clutch is worn.

If the problem is accentuated whenever your engine tends to be lugging at lower RPMs and especially going up hills where the engine is under heavy load, then you should first check your ignition tuneup items like spark plugs, spark plug wires and distributor cap and rotor if your car has those components.
