Who can my son use to sign his photo for his provisional driver's license?

his doctor or teacher I am sure can sign the photo.
if the worst comes to the worst in two years or less someone off the list will have known him long enough.
could he not go to his old school an ask a teacher or headmaster?

anyone form his schoo/college, principal, teacher , that knows him,, can sign it,, doctors used to but, some have stopped as it 'takes to much time up' . if he knows his local post master he could sign it if he is on friendly terms,, hope this helps.

Teacher/Lecturer? If he works and his boss or whatever has known for more than 2 years have them to do it.
