When applying for a 70+ license what documentation is needed?

You fill in a form you can get from the Post Office or on line. You need to make a declaration that your health is good enough to let you continue to drive. If this is your first photocard licence, you also need to supply a passport style photograph and proof of identity. A main Post Office will normally do the ID check for you so you don't have to post off your passport to Swansea.
I think you need a doctors certificate to prove you are healthy enough to drive and not a danger to other road users or pedestrians.
You would need something from your doctor and insurance company. I was told at 60 I could no longer drive. To me I figure the road and the public are just better off without me driving. It upset me a little at first but I have gotten over it.
