Can i drive if i have lost my provisional and i have changed address?

I would be very careful, if you are stopped and given an order to present your license and details, you won't be able too - so probably best to do what you can to sort it out sooner rather than later, just in case.
Your details will still be on computer so if you are stopped it can be checked, i advise you sort it out soon though as you have changed address

it is an offence not to tell the dvla about a change of details.
as long as you have informed the dvla about your change of details, then you will be fine, and they will send you a replacement license in a very short time. but the police take a pretty dim view if you do not have an upto date license.
I would get onto the DVLA if I were you.

Seriously, you need to get a duplicate copy of the provisional license, and change your address with the DMV.
you can drive but you commit a technial offence if you have not reported the new address - the longer you wait the more likely it is you will be prosecuted if ever it comes to the notice of the police

Apply for a replacement license. But in the meantime call the DVLA and tell them you've changed your address and will be applying for a new license under your new address. As long as you have informed the DVLA you can drive and won't get into trouble as you've done everything physically possible.
You should get a D1 Form and tick the box you ahve lost/stolen lience and where it says has your name/address changed since last licence -fill in your old address.
It does state clearly on your licence that you must notify the DVLA Immediately of change of address or health. or you may face a fine. Also if you are pulled over and are asked to provide your license and fail to do so you will again be prosecuted.
