A wheel fell off our camper 500 miles after last changed - possible or was it tampered with?

I'm assuming that the nuts were gone and not that the wheel sheared off at the axle and the hub was still left inside the rim, so.
It's more than likely that when the wheel was put back on, the nuts were tightened finger-tight, and then never tightened the rest of the way with a lug wrench or imact wrench.
Sure it's possible that they were tampered with, but unless someone really doesn't like you I'd say that's pretty unlikely.
Always give them a visual inspection at the very least before you pull out.

If your lug nuts weren't tightened all the way it is possible it fell off. After changing a tire always be sure your lug nuts are tightened all the way bc they will slowly work their way off.

Quite possibly, A camper is in essence a commercial chassis & have heavy duty wheels & fixings, 5 or 6 studs, it is recomended that after a wheel change nut should be checked after 50 miles or so. Surely you must have noticed a change in road handling before it became detached, studs wouls have separated one by one not all at once.Sorry bad luck.
