if it is the SIM PIN code you have forgotten, you will need to call or visit your mobile phone operator and obtain the PUK (PIN Unblocking Code). They will also guide you through the steps on unlocking the PIN.
You will need to provide proof that you are the SIM Card owner - they'll probably just verify your address.

i don,t quiet understand your question if you mean your puk code go online and download it or ring your service provider, if you mean the code that is the one that they ask you for when say you have recieved a blutoothed message it says enter code it is 0000 unless you have changed it and you have forgotten it you will have to ring your service provider and they will help you but only after they establish it is your phone i have samsung D500 and a samsungD600 and the codes are 0000 ( unless you mean your pukcode this is on your box it came in or you have to contact your service provider
You need to lock your phone basically when it starts asking for a PUK code. Contant your mobile operator and they will perform a full secruity check and provide your code. Make sure to write down your PUK code, because if you lock your phone again or forget your pin number the PUK code would be exactly the same, saves you ringing up again asking for it.
