Not sure what your question is. Molarity is a measurment of concentration in moles per litre. So if blood glucose is 5mM, there are 5 milli moles per liter of blood.
If you are dialising blood you would have the desired concentration in the dialysis liquid to prevent dropping the blood glucose levels.
