Here's the CORRECT answer. For starters, it's not illegal to have scrap silver in the US. If you're looking for someone to refine it for you, look up a business called Silver Towne in Winchester, Indiana. They will take your scrap and convert it into ingots (for a fee of course, as would any smelter, so make sure you have enough scrap to make it worthwhile to do it). They also might buy your scrap outright and then just trade for the equivalent in their pre-made .999 silver bars, which might be the most convenient and quickest way to go. Their site is silvertowne.com and their phone # is 800-788-7481. There are other refiners like Engelhard, National etc, but I've found these guys very easy to deal with. Hope this info helps.
Most likely you would have to go to a specialty Jewelery shop where they make custom designs. You can do a google search for the one nearest you.
Br careful, as without a license you cannot hold bullion and silver in unworked state could be described as bullion not jewellry. I know 'cos it happened to me.
