bleach and a good scrub. If it is too bad you might have to chip the grout out and start again.
Clorox Bleach Spray is the best product to use.
Tilex, spray it on, wait, rinse! No scrubbing!
There are special tub & tile spray cleaners at your local store.
Careful how you use this stuff, it's pretty nasty.

Mold will disappear mostly with ordinary bleach, but the urine is in essence a true stain--so you will probably have to dig out the old grout and replace it. Fortunately re-grouting is easy and cheap. Use a GOOD tool for the job to remove the old grout to prevent chipping your tiles.
Change you tiles and try not to piss on the new ones.

What makes the job more difficult is that even when you scrub the grout, some of the dirty water sinks right back into the grout. Scrub the tile and grout, then use a wet-vac to pick up the dirty water. Bleach is good, just make sure to dilute it if you are using it straight. (4/1)
Try peroxide, the same stuff you use on cuts. It is a great stain remover.
