When you first opened the pen, you primed it, right?
You do that by cleaning it, attaching a needle, dialing it to 37.5, pulling back on the knob at the end until it clicks, and then pushing on that knob and you should see a small amount of medicine at the end of the needle.
Then, throw away that needle, clean the threaded end again, and use a new needle. Then, dial your dosage (150) and pull back on the knob again until it clicks. You'll know it's working if both the dial says 150 and the knob at the end shows 150 with an arrow. Then you're ready to inject the medicine. If the knob doesn't show 150 ( the same amount as you've dialed in), either the pen doesn't have enough medicine in it (likely) or the pen is malfunctioning, and you'll need to use another one. I've used lots and lots of these pens, and I've never seen one malfunction, but I'm sure its possible. If you push on the knob and no medicine seems to inject, remove the needle and prime the pen again. If you can't get droplets of medicine to appear at the tip of the needle when you prime it, you'll need to use a new pen.

(Oh, and also -- don't use the pens straight from the refridgerator. Warm then up in your hand if you haven't set it out the day before you use it. Otherwise, the medicine will really sting!)
