what the reasoning behind this saying. Is this the basis of the police investigating medical staff or carers, when 3 identical or similar incidents occur under their stewardship.

I'm not familiar with the phrase, but I should imagine that when the same error is made more than 2 times, it starts to look suspiciously like that person isn't doing their job correctly and may be taken aside to discuss it.

i have no idea the reason behind it..Maybe you can make any mistake you want one, twice is carless but to make the same mistake three times is a bit daft? As humans shouldn't we be learning from our mistakes?
