Self-awareness is just being aware of yourself, your needs, wants, things you want to fulfill. I think it's like the top of the pyramid scheme of needs.
Self evaluation is looking at yourself and finding out how you've been going along with everything, how you deal with certain emotions and situations.

Self awareness for me is being aware of all the things i do, i think, and say. In the moment. Not as analyses or retrospection.

Self evaluation is when im not aware and am judging my self and my actions against the conditioned response's which are deemed right or wrong.
Evaluation is measuring up to...comparison, imitation, fear, or simply to evaluate exactly what we mean by that word self.

To evaluate is to consider the value of - therefore self-evaluation is to consider the value of yourself, i.e. to think about your strengths and weaknesses, compare yourself to others, judge yourself and so on. I would think that to be self-aware you need to engage in some form of self-evaluation.
