This is actually a great question.
Some, I think, have their reasons for being afraid: e.g. one person labeled paranoid who's a drug-addict and owe a lot of money to some not-too-nice-guys. Well, I can understand that he/she is afraid of them.
Other people might have rather unrealistic thoughts of who's coming to get them, e.g. the FBI, midgets or their neighbor. If they do not have a good reason for this, you're paranoid. "Not a good reason" is usually thought to be that most people would disagree with the basics in you're story, e.g. "The Indian medicine man is going to kill me because I ate tomatoes today" (This isn't waterproof as the others might be wrong).
I guess if you accept the basics of the story/thoughts of a paranoid person, there's no problem understanding the fear.

You could act weird because you feel like you are being watched. If so, they will start watching, and out you go!!
