How do the Premiership champions get decided if..?
the 2 top teams are level on points and goal difference, home games and away games?

I think its head to head & then a play -off
not sure, maybe like champions league were it goes by results against each other.

The FA do not have a documented policy on this one (due to the relatively low chance of it happening).
Other football competions involving group stages (where the lower number of teams make it more likely to occur) usually go on the results between the two teams in questions.
However some sports like cricket will use the 'pieces of paper in a hat' method.
The team with highest goals scored wins it. Both teams could have identical goal difference but with different numbers of goals scored and conceded. - If this is included in your equation then they would have a play off decider - more money for sky!
