For a 50th birthday I would like to arrange a weekend break in the UK in a cottage/house with a games room, jacuzzi and maybe indoor pool. I would also like to have butler service provided - breakfast prepared and at least one evening meal cooked and served. Any suggestions of somewhere within a couple of hours drive from Surrey.

For a nice cottage break try the National Trust - they have some cottages which they let out. It's not going to have all of the extras that you're after but it will be in gorgeous surroundings.
