Answers:
If you are travelling on a British Passport you will be allowed entry under the Visa Waiver Program. See this site for more information http://www.travel.state.gov/visa/temp/wi.
look on the usa embassy website
yes! you need to have the US visa to enter into USA since you are not a US citizen.
Contact the Emabssy or Consulate for better information.
On flights to the US, British citizens get a Visa Waiver (green card) application which grants you access for upto 3 months - you go through customs, answer a few questions and it is put in your passport.
If you were passing through Fiji, this should still apply. If not, you'll have to get one once you reach the airport, I imagine.
No. You'll get an automatic 90-day travel visa upon entering.
If your passport is machine readable, valid for more than six months, and you have filled in the requisite form issued in the plane, you will be allowed in under the visa waiver scheme for three months. Your photo and a fingerprint may be taken. Old style (blue) passports are not acceptable.
The American Embassy in London gives details on the website.
Make sure you have you valid passport (must be valid for 6 month after you leave the US) and fill in the green Visa Waiver Form they give you on the plane ( not to confuse with the "Green Card"). Have your onward ticket ready and go. If you wouldn't be allowed in the US the airline wouldn't take you there. Happy travels.
yes and they'll take your fingerprints too
at arrival
